• thorough review of Full intake gathering recent prescriptions, health
history, diagnosis, abilities to conduct activities of daily living, care needs
and most pressing concerns
• virtual intake review with client and healthcare proxy or other family
member (plan for approximately an hour) to discuss all relevant
information to discern appropriate options
• Free access to Iaso which allows patients to update loved ones,
their health care proxy, patient advocate and community doctor
with health information while hospitalized or in a healthcare facility.
